Maple Leaf Award Nomination Form

The Maple Leaf Award recognizes women and men who are making a difference in our world. You are welcome to submit more than one nomination. Two awards will be presented during Alumnae Weekend.

Criteria:
All Mount Saint Joseph Academy and College alumnae, associate alumnae and faculty may nominate.
Nominees must be alumnae or associate alumnae of Mount Saint Joseph Academy or College.
Nominees must personify the values of their Mount Saint Joseph education in contributions to their local faith communities, civic communities, and/or Mount Saint Joseph.

An anonymous panel of alumnae will choose the winners.
